What is USMLE Step 1?
The United States Medical Licensing Examination (USMLE) Step 1 is the first of three exams required for medical licensure in the United States. It assesses understanding of basic medical sciences with an emphasis on principles and mechanisms underlying health, disease, and modes of therapy.
Step 1 became a pass/fail exam in January 2022. Numeric scores are still reported to the examinee on request but are no longer visible to residency programs. This shift has elevated the importance of Step 2 CK (Clinical Knowledge) for IMG residency applications.

Since Step 1 is pass/fail, Step 2 CK score has become the de-facto differentiator for residency applications. A 240+ on Step 2 CK opens doors to most specialties; 250+ is competitive for radiology, dermatology, orthopaedic surgery, and other top-tier specialties.
IMG-specific advice
- Step 1 attempt limit: 4 attempts (down from 6) since 2024. Take it once, take it right.
- Clinical rotations (USCE): Recommended 3-6 months of US clinical experience before applying to residency.
- Letters of Recommendation: Aim for 3 USCE LORs from US physicians.
- Visa: Most IMGs match on J-1 or H-1B visas. Plan finances accordingly.
